Conviva Senior Primary Care runs an Integrated Home Care Program for high risk patients who need medical care at home. The goal is simple: keep people out of the hospital by bringing excellent care to where they live. You'll work in the Corpus Christi, TX area and report to the National Medical Director for In Home Care. This isn't a desk job. It's a role where you get in your car, visit patients in their homes, and actually see the difference you make.

What you'll actually do

• Go into patients' homes and do thorough evaluations that cover their medical, functional, and psychosocial needs. You're looking at the whole picture.
• Work closely with each patient's primary care physician to develop personalized treatment plans. It's a co management model, not a solo gig.
• Educate patients and their families about conditions, treatment options, medications, and how the program works. The goal is to help them take an active role in their own care.
• Handle acute medical issues right there in the home ; things like pneumonia, CHF or COPD exacerbations, UTIs. You diagnose and treat on the spot.
• Administer and monitor medications safely, following established protocols.
• Document everything in the EMR system (Athena, Q4 2025). Every home visit, assessment, and intervention needs to be recorded thoroughly.
• Join regular team meetings on a rotating basis to discuss cases, share insights, and plan care together.
• Carry a caseload of about 45 patients per month, roughly 200 unique patients per year. That works out to 6 to 8 visits per day with a weekly or every other week visit cadence.
• Take after hours call on a rotating schedule. When you're on call, you handle telemedicine consultations for patients and families as needed.

What you need

You need to be a Nurse Practitioner or Physician Assistant with a Master's Degree in Nursing. You must have a Texas license. And you need at least 5 years of experience in acute care, geriatrics, home health, or a related field. On top of that, you should be comfortable using an EMR system for documentation.

Schedule

Regular hours are Monday through Friday, 8:00 AM to 5:00 PM. The after hours call is shared fairly among the direct patient care team. Scheduled weekly hours are 40.

Pay

The starting base pay for full time (40 hours per week) ranges from $123,800 to $170,300 per year. Where you land depends on geographic location, your skills, experience, education, and certifications. This role is also eligible for a bonus incentive plan based on company and individual performance.
