
About This Role

What you'll do
You're the nurse implementing the plan. You'll manage direct patient care in the home, following the provider's orders and staying within your nursing scope.
- Monitor and document everything; vitals, blood glucose, other specimens; and report any changes.
- Medication administration is a big part of it; that includes insulin and IV fluids, with thorough documentation every time.
- Wound care is on you. Inspecting, changing dressings, and helping with personal hygiene all fall under your watch.
- You need an eye for new issues, too. Assessing for injuries, infections, or bedsores and starting the right treatments is key.
- It’s not a solo act. You’ll collaborate with providers, aides, and other health pros to make sure the patient’s outcomes are as good as they can be.
What you need to bring

- A current, unrestricted LPN license for the state where this job is located.
- Your CPR/AED/BLS/First Aid certification needs to be current.
- You have to have reliable transportation to get to and from work locations and patient homes.
A note on experience: if you've got practical trach or ventilator care under your belt, that's a definite plus. But it isn't a strict requirement if you don't.
