About This Role
The Job
Capital City Hospice, part of the Addus family of companies, is looking for an LPN. We're an Ohio based hospice organization that puts patients and families first. Here's the thing we've learned: you can't provide great care if you're burned out. So we built roles with low caseloads, no routine on call, and a culture that actually respects your time.
The Schedule
You'll work Monday through Friday, 8 a.m. to 5 p.m. And get this: only two holiday on call shifts per year. That's it.
What You'll Do
- Provide direct patient care under the State Nurse Practice Act
- Complete comprehensive assessments and follow the patient's individualized care plan
- Document patient services accurately and on time
- Educate patients and families about diagnoses and care plans
- Collaborate with the interdisciplinary team ; aides, OT, PT, MSW, Dietitian, and Clinical Supervisor
- Manage pain and symptoms effectively and evaluate how patients respond
- Support patients and families with real compassion, professionalism, and respect
What You Bring
- Graduate of an accredited practical or vocational nursing program
- A current LPN license in the state
- At least 1 year of medical or surgical experience (home health or hospice experience is preferred)
- Current CPR certification
- A valid driver's license and a reliable, insured vehicle
