About This Role

What You'll Be Doing as a Care Manager (RN)

BoldAge PACE runs an all inclusive program of care for older adults who want to stay living at home with dignity. As our Registered Nurse Care Manager in Columbus, OH, you'll be the clinical anchor for a group of participants, making sure their medical needs are met across the clinic, the center, and their own homes.

You'll work as part of an interdisciplinary team ; the IDT ; to build care plans that actually fit each person's life. You'll assess what they need, provide nursing care, and follow up to see if it's working. You'll delegate tasks to CNAs and home care aides based on what each participant's plan calls for. You'll also talk regularly with primary care providers, clinic staff, and other team members to keep everyone on the same page.

Your week could include seeing someone in the clinic, visiting a participant in an assisted living facility, or stopping by a home to check on a chronic condition. You'll be part of a 24/7 on call rotation for triaging participant needs. And every month, you'll reconcile the medication administration records (MARs) for your assigned panel of participants to make sure meds are accurate and being taken as prescribed.

When abnormal results come back, you'll notify providers. When test results are normal, you'll tell the participant. You'll teach participants, their families, and caregivers about medications, self care, infection control, and safety. You'll also review and revise care goals as things change, working right alongside the participant and their family to keep the plan current.

You'll attend all IDT meetings, utilization review meetings, and quality improvement meetings. If needed, you can step in as a backup for the Home Care Coordinator role on the IDT. You might also perform duties assigned to other IDT members if you've got the license and competency for it. Plus, you'll help evaluate the competence of CNAs and home care aides and delegate tasks to them.

There's also documentation: nursing assessments need to be complete, timely, and aligned with policies and regulations. You'll implement nursing interventions that match the care plan. And you'll follow all PACE policies, OSHA safety guidelines, infection control protocols, and privacy rules.

Other duties will come up as needed. That's part of working in a program that's built around the person, not the schedule.

What You Need to Have

  • An active RN license in the state where Columbus, OH is located (if that's New Jersey, you must be licensed by the New Jersey State Board of Nursing)
  • A Bachelor of Science in Nursing degree is preferred, but not required
  • BLS certification is required
