Oak Street Health is looking for a Behavioral Health Specialist to work out of our clinics in Colorado Springs, CO. This is a full time role, 40 hours a week. You'll hold an active LICSW, LPC, or LMFT ; that's non negotiable.

The job mixes direct patient care with care coordination. You're not just doing therapy; you're the behavioral health resource for an entire primary care team. Depending on which clinics need you and how mature their patient panels are, you might cover more than one location.

What you'll actually do

  • Work alongside primary care teams to figure out which patients should be referred into Oak Street's Behavioral Health Program
  • Screen and assess patients for common mental health conditions and substance use disorders
  • Write care plans and either provide treatment yourself or connect patients to Oak Street's psychiatric consultant or outside resources
  • Deliver short term counseling using evidence based treatments
  • Coordinate closely with the psychiatric consultant ; think warm handoffs for telehealth visits and shared planning on patient care
  • Build relationships with the primary care teams and keep them in the loop whenever a behavioral health plan changes
  • Document every patient encounter and counseling session in Oak Street's electronic medical record, accurately and on time
  • Help shape the broader Behavioral Health Program by showing up to workshops, giving feedback through surveys, and participating in regional efforts
  • Tackle other duties as they come up ; it's a clinic, stuff happens

A note on remote work

The role has potential for some remote days down the line. That depends on whether you hit and keep program metrics. No guarantees up front.

What you need to bring

  • A Master's Degree from an accredited school in social work, counseling, marriage and family therapy, or another mental health field
  • The highest clinical level of license your state offers (LICSW, LPC, LMFT or equivalent)
  • Experience screening people for common mental health disorders and substance use issues
  • Experience assessing those conditions and building treatment plans around them
    Strongly preferred:
    A real working knowledge of brief structured interventions ; Motivational Interviewing, Behavioral Activation, Cognitive Behavioral techniques ; enough that you can deliver them confidently
You know how to tell apart the differential diagnoses for common mental health and substance use disorders
    A nice plus:
    Solid PC skills
US work authorization is required.
