Registered Nurse

The Role

Hospice care is deeply personal, and the people who coordinate admissions set the tone for everything that follows. Crossbridge Hospice is looking for a Registered Nurse to own the admissions process in Cincinnati, OH ; from that first referral through to the moment a patient is on service. You're the person who makes sure nothing falls through the cracks, and you're the calm, knowledgeable voice families remember.

What You'll Do

This role lives at the center of referrals, documentation, and direct communication with patients and families. You'll juggle a lot, but you won't be doing it alone.

• Receive and process referrals from Community Liaisons, physicians, patients, families, and community partners
• Coordinate requests for patient records and schedule appointments for informed consent and RN assessment visits
• Verify and upload patient records to the EMR
• Notify the assigned interdisciplinary team and ancillary providers about new referrals, admissions, and non admissions on time
• Coordinate supply and DME provision to meet patient needs
• Follow up on patients who haven't been admitted or who've been discharged
• Maintain and organize patient charts, making sure all documentation is complete, signed, and dated
• Transfer completed admission charts to the Team Coordinator promptly
• Explain hospice services, benefits, and eligibility to patients, family members, and referral partners as needed
• Keep team members informed of scheduled admission appointments and updates
• Communicate identified needs, issues, and potential solutions to your supervisor
• Attend staff meetings, educational programs, and community events as requested

What You'll Bring

This isn't a role for someone who wants to stay behind a screen all day. You need to be comfortable on the phone, in conversation, and in the field.

• Active, unrestricted Registered Nurse license
• Availability during agency operating hours and as needed
• Sound professional clinical judgment
• Strong verbal and written communication skills
• Comfort and proficiency with EMR and data entry systems
• Hospice experience is a plus, but not required
