The Job in a Nutshell

This is a full time Visiting RN position with a home care agency that's been part of the NYC metro community for over 50 years. They're highly respected, and they're offering a $15,000 sign on bonus to the right person.

What You'll Actually Do

You'll manage every aspect of patient care that happens in their home. That means conducting visits, coordinating with other disciplines, and making sure everything stays connected.

• Work directly with patients, physicians, RNs, and other health professionals to deliver quality care.
• Partner with patients and caregivers to build an individualized plan of care ; one that promotes healthy lifestyles, closes gaps in care, and cuts down on unnecessary ER visits and hospital readmissions.
• Develop and share health education and consultation with community members, clients, and families.
• Handle client emergencies by notifying the right people quickly.
• Complete all required documentation. That includes communicating with primary care providers, completing 485 forms and interim orders, updating medication profiles, and reviewing case communication notes.
• Use Homecare Homebase EMR daily to review follow up items and any incomplete documentation.
• Participate in case conferencing with your supervisor.

What You Need

You have to be solid on Medicaid/Medicare regulations ; that's not negotiable here. Beyond that you should be comfortable managing patient cases entirely from home visits. This role expects you to take ownership of each case without needing constant hand holding.

The Pay

$105,000 to $125,000 per year depending on experience. Plus a $15,000 new hire bonus when you join.
