Crossbridge Hospice is looking for a Case Manager RN for Cincinnati, OH. This isn't just a job. It's about being present for people at the end of their lives, and for the families who love them. What You'll Actually Do
- Take primary responsibility for a patient/family caseload during on call hours. That means assessing, planning, implementing, and evaluating care needs while collaborating with the triage team
- Report on call activities to clinical leadership and Branch RN Case Managers at the end of each call shift
- Maintain and complete care coordination notes for visit notes and other clinical information
- Help get medications and supplies delivered after business hours, on weekends, and holidays
- Attend interdisciplinary team meetings during your on call period for full team discussion and evaluation of patient/family needs
- Admit patients eligible for hospice services under a physician's order, working with the triage team
- Report patient status and the need for other disciplines to clinical leadership, the attending physician, and the hospice physician
- Develop patient care plans that address identified problems and set mutually agreed upon goals with patients during on call hours, weekends, and holidays
- Use the 4Ms ; What Matters to the patient, Medications, Mentation, and Mobility ; to provide Age Friendly Care
- Direct, supervise, and instruct hospice aide staff in providing personal patient care
What You Need
You have to hold a current, unrestricted RN license in the state where you'll practice. A valid driver's license and proof of auto insurance are required too. You've got at least two years of RN experience. And if you have two years of hospice or palliative nursing experience, that's preferred.
Schedule
This is a per visit role. You'll need to pick up 3 shifts each month. Hours run Monday through Friday, 8 am to 5 pm.
