About This Role

Alright, let's talk about this RN role with JourneyCare Home Health. We're part of the Addus family, and we need a nurse who genuinely likes the autonomy of field work. This isn't about a hospital floor; it's about managing your own caseload, driving around Chicago, IL and the surrounding towns, and being the key clinical point person for your patients right where they live.

The Work

You'll handle everything for your assigned patients from start to finish. That means doing the initial assessment to see if home health is the right fit, admitting them, creating their care plan, providing the hands on nursing, and evaluating their progress. Your day is built around visits ordered by the Clinical Services Manager.

  • Your assessments have to look at the whole picture; the physical needs sure, but also psychological, social, and spiritual factors that affect a patient and their family.
  • You're the connector. You'll be on the phone with attending physicians, pulling in other members of our interdisciplinary team, or coordinating with outside agencies to make sure care isn't fragmented.
  • The plan of care isn't static. You'll revisit patients regularly to see how they're doing and update their plans accordingly.
  • Perform the skilled nursing procedures; therapeutic, preventative, rehabilitative; that the physician directs.
  • For our RNs with the appropriate licensure, this includes providing clinical pronouncement of death when necessary and supporting the family through that moment.
  • Keeping clear, accurate, and timely records is non negotiable. We're talking charts, progress notes, reports, and those care plans.

What You Need to Bring

A few boxes you've gotta tick. First off, you need to be a graduate from an accredited registered nursing program and hold a current license in good standing here in Illinois.

  • We're looking for at least one year of clinical nursing experience under your belt. If that experience is in a setting like intensive care, rehab, or medical/surgical nursing? Even better.
  • Previous home health experience is a definite plus; you already know the rhythm of this work.
  • A valid CPR certification that you'll keep current.
  • This job is on the road. You must have reliable transportation, a current driver's license, and appropriate automobile insurance. We do reimburse for mileage.

This is a full time field position based out of Chicago, IL. The salary range is $80k to $105k annually; where you land in that range depends directly on your years of relevant experience.
