What You'd Be Doing
The core of the role is managing a panel of patients, capped at 450. You're the quarterback for their health. That means full accountability from assessment and diagnosis to treatment plans and follow up.
- You'll act more as a long term health coach than just a consultant for sickness. Helping patients set goals, checking in on progress; that's central to the model.
- When your patients are hospitalized, you're expected to engage with the hospitalist team, whether they're with ChenMed or not. You stay involved.
- A big part of the job is simplifying complex care into actionable steps, both for your patients and for your own care team.
- You'll lead a small team that includes a care promoter (their term for a medical assistant), a care facilitator, and a care coordinator.
- For patients who can't come to the office; those who are homebound or in a skilled nursing facility; you'll work with a transitional care team and other community resources.
- You'll help cover for other providers when needed and play an active role in managing the day to day at your center.
The Specifics They Need
This isn't just about having an MD. They have a specific way of working that requires certain credentials and mindset.
- A Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree is required.
- You need an active, unrestricted medical license in the state where you'll be practicing.
- Board certification in Internal Medicine or Family Medicine is mandatory; board eligibility won't cut it.
- A current DEA registration is non negotiable.
- They measure success through specific metrics: patient admissions per thousand, closing clinical care gaps, patient experience scores (CGCAHPS), and managing medical costs effectively.
- The schedule involves using between 18 to 21 appointment slots daily. New patient visits count as two slots, follow ups as one slot.
