Hey there. I'm putting this listing together for JourneyCare Home Health, which is part of the Addus family. We need an RN to cover a territory in Chicago, IL and the surrounding areas.

The Work

This is a field based position. You'll manage your own caseload of patients, providing skilled nursing services and coordinating their care so they can recover at home.

• Your visits are assigned by the Clinical Services Manager. What you do on those visits depends on your licensure.
• It starts with a full assessment; gathering data on the physical, psychological, social, and spiritual factors that could impact the patient and family.
• You're the point of contact. That means initiating communication with the attending physician, the interdisciplinary team, and any other involved agencies to coordinate care.
• You'll perform therapeutic, preventative, and rehabilitative nursing procedures as directed by the physician.
• For applicable licensure, clinical pronouncement of death and supporting the family is part of the role.
• Keeping meticulous records is non negotiable. You'll maintain accurate patient charts, progress notes, reports, and prepare care plans in a timely manner.

What You Need to Bring

A few things are essential for this job.

• You must be a graduate from an accredited registered nursing program and licensed in the state.
• A valid CPR certification is required, and you need to keep it current.
• You have to have reliable transportation, a current driver's license, and appropriate automobile insurance. This is a driving job.

A bit of experience goes a long way here. At least one year of clinical practical nursing experience is what we're looking for. If that experience is in intensive care, rehabilitation, or medical surgical nursing? Even better. Previous home health experience is also preferred.

The Details & How to Apply

The salary range for this position is $100,000 to $120,000 per year. That offer will be based on your years of relevant experience.

We have both full time and PRN schedules available; we can talk about what works best.
