The Work
This is a field position on our Crisis Care team. You'd be following the plan of care for patients receiving continuous hospice support, providing treatments, and working with the team to help patients and their families.
Your day to day would involve a few key things.
- Provide direct patient care within the scope of the state's Nurse Practice Act.
- Carry out nursing practice after an assessment and according to the established plan.
- Your documentation needs to be accurate and timely, reflecting that plan of care.
- Assess what patients and their families need to know about diagnosis and care, then provide that education.
- You're part of coordinating hospice services, so you'll report identified needs back to the interdisciplinary group.
- Manage pain and symptoms appropriately.
- Evaluate how patients respond to treatments and medications.
The Schedule & Territory
You'd work Monday through Friday, three 12 hour shifts from 7 AM to 7 PM. You're also on every other weekend. The territory is based in Chicago, IL (Chicago, IL 60618).
What You Need to Bring
A few non negotiables here. We need someone who gets it.
- A graduate from an accredited practical nurse or vocational nursing program.
- Three years of nursing experience under your belt. Community health, hospice, or medical/surgical background is a definite plus.
- A current LPN license for the state where you'll practice.
- Current CPR certification is required.
- A valid driver's license and an insured car that meets state requirements. You'll be driving.
- Good verbal and written communication skills matter. So does being organized.
- You have to understand the hospice philosophy and what terminally ill patients really need. It's central to everything we do.
The Pay
The hourly rate is between $26 and $38. Where you land in that range depends directly on your years of relevant experience.
