About This Role
Alright, let's talk about this job. It's not your typical hospice role. We need someone who can bridge the gap between a hospital and our hospice services, acting as a clinical consultant right there in the facility. You're not working from home; you're embedded in assigned hospitals in the Chicago, IL market, reviewing charts and building relationships to ensure patients who need us find us sooner.
What you'll be doing
You'll serve as the hospice expert for hospital care teams. Your main focus is identifying patients who could benefit from hospice by reviewing clinical documentation and EMR systems. You'll build patient profiles, spot decline patterns like frequent hospitalizations, and then consult with doctors, nurses, and discharge planners.
- Educate hospital staff on when hospice is appropriate.
- Collaborate with our field sales reps to set up meetings with external care teams.
- Build and maintain workflows for patient assessments, education planning, and meetings.
- Look for trends and partnership opportunities to get patients into hospice care earlier.
- Foster trusted relationships that improve patient outcomes and end of life satisfaction.
- Communicate clearly with hospital leadership and care coordination teams.
- Organize, track, and prioritize comprehensive patient reviews.
- Meet productivity requirements for timely record review and provider education.
The kind of person we need
We're looking for a clinically trained professional with business development experience. Physician sales experience is a big plus. Hospice experience isn't required; we can teach you that part. What you really need is drive, curiosity, and resilience.

Formal qualifications
- A Bachelor’s degree in marketing, business administration or a related field is preferred. An acceptable combination of experience and/or training may be considered instead of formal education.
- A state Registered Nurse license is preferred but not mandatory if you have other relevant clinical training/experience..
