Alright, let's talk about this job. I'm recruiting for a Home Care Registered Nurse position with VNS Health in Brooklyn, NY. This isn't hospital nursing. You're going to someone's home or a community facility, and you're the one calling the shots on their care plan. It's for nurses who want real autonomy and a direct, lasting impact on how people heal and age.

What you'll do

You'll be the primary nurse managing patient care out in the community. You're not alone; you're part of an interdisciplinary team; but you're practicing independently. Your main focus is creating and delivering highly personalized nursing and care management.

Develop individualized care plans for patients in their homes.
Your patient's needs aren't static. You'll constantly assess them and adjust the plan of care as things evolve.
This work directly helps keep people out of hospitals, letting them recover where they're most comfortable.

What we need from you

The non negotiables first. You must have a current New York State license to practice as an RN. We also need you to have at least one year of prior nursing experience in a medical/surgical environment.

You'll need a valid driver's license to get around. If you don't drive, a New York State Non Driver photo ID card might work instead; we can confirm that during the hiring process.

A quick note if hospice care is your path: wound care certification is required, but if you don't have it yet, we prefer you're willing to get it within your first year here. Also, certification in Hospice and Palliative Care Nursing is a nice plus.

The schedule & pay

This posting is for a per diem, per visit role. Your pay will be based on each visit you complete. The hiring range for that is $78.64 to $123.20 per visit.
