You're a Physical Therapist. Your office is your car and your patient's living room. The goal isn't just to treat; it's to keep people out of the hospital by healing where they're most comfortable. You design the program, you deliver it one on one, and you get to see the real world progress because you're in their home. It's a different kind of accountability and reward.
What you'll do
You'll manage your own caseload in the community, working as part of an interdisciplinary team but making your own clinical calls.
- Perform comprehensive assessments to determine patient needs and develop full treatment plans with expected outcomes.
- The therapy itself is delivered personally, in the patient's home or care facility.
- Your plan isn't static. You'll constantly evaluate how the patient is doing and adjust the course of care as their needs evolve.
What you need to have
- A New York State License and current registration in Physical Therapy. Non negotiable.
- At least one year of PT experience. If you don't have that year, they'll consider candidates who successfully complete a probationary period as a VNS Health Rehab Intern.
- A valid driver's license is required if you're working anywhere outside Manhattan.
Schedule & Pay
This is a full time role. The annual base pay range is $85,000 to $127,500 per year.
They also listed per visit rates if you're looking at a per diem version of the position. It's location based:
- Most areas (everywhere except the Bronx and specific parts of Brooklyn): $70 to $75 per visit.
- The Bronx: Weekday visits are $95; weekend visits are $100.
- Parts of Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint only): Same as the Bronx - $95 on weekdays, $100 on weekends.
