Nurse Practitioner

What you'll do as a Nurse Practitioner at VNS Health

This is a clinical leadership role. You'll manage the full scope of advanced nursing practice for a targeted patient population in Brooklyn, NY, with home visits in Manhattan. You'll work under the supervision of the Clinical Director for the Nurse Practitioner Program.

You'll provide comprehensive advanced nursing care. That means physical exams, taking detailed histories, screening for physical and psychological conditions, handling emergent interventions, pharmacological and non pharmacological treatments, ordering DME, prevention activities, care management, referrals, discharge planning, counseling, and patient education.

You'll build treatment plans based on your clinical findings. You'll decide when a collaborating physician, a specialist, or emergency care is needed. And you'll collaborate closely with patients, their families, primary care doctors, and other team members for assessment and care planning.

You'll manage care across different client settings ; intensive, continuous, whatever the patient needs. You'll evaluate how patients respond to therapy and interventions. When the plan isn't working, you'll revise the inter professional plan of care to hit quality outcomes.

You'll spot when new or revised clinical protocols are needed. You'll collaborate with physicians and others to develop those protocols and provide training. You'll also make sure clinical services comply with Patient Centered Medical Home standards, meaningful use of medical record data, and quality measures like HEDIS and QARR.

What you need to qualify

Licenses and certifications:

• License and current registration as a Registered Professional Nurse in New York State
• Certificate and current registration as a Nurse Practitioner in New York State, with a specialty in adult health, family health, or gerontology
• A valid driver's license may be required depending on operational or regional needs
• You must maintain credentialed status with VNS Health Medical Care at Home and associated managed care plans
• You must be certified by ANCC or another accrediting Nurse Practitioner body to bill Medicare and meet credentialing requirements
• NPI, Medicaid, and Medicare provider numbers are preferred
• A collaborative practice agreement with a physician (per New York State regulations) is preferred
• For Psychiatric Nurse Practitioners only: current PMHNP BC certification is required

Education:

• Master's Degree of Science in Nursing, or another graduate degree from an NP educational program registered by the New York State Education Department as qualifying for NP certification
• Current ANCC or AANP certification as an adult, family, or geriatric nurse practitioner
• For Psychiatric Nurse Practitioners only: Master's Degree in psychiatric mental health Nurse Practitioner (PhD preferred)

Experience:

• At least two years of experience as a nurse practitioner using your full scope of practice is preferred
• Clinical home care experience or two years of managerial experience is preferred
• Demonstrated knowledge of HEDIS and QARR quality measures, ICD 10 and CPT coding for reimbursement of services is required
• Bilingual skills may be required based on operational needs

Pay range

USD $109,900.00 ; USD $146,500.00 per year.
