About This Role

Alright, let's talk about this hospice RN position in Brooklyn, NY.

You'd be managing a caseload of terminally ill patients, building and following their care plans to maximize comfort and quality of life. The work happens where the patients are: in their homes, skilled nursing facilities, or inpatient units. You're the one coordinating everything between LPNs, home health aides, and family caregivers for your assigned patients.

A big part of it is doing those physical and psychosocial assessments to figure out what the patient and family really need, then shaping the hospice plan of care around those goals. You'll also handle ordering medical equipment and supplies, making sure things get delivered before weekends or holidays, and keeping track of what's in the patient's home.

It's field based work covering Queens. Your schedule is Monday through Friday from 8:30 AM to 5 PM.

What you need to bring

  • A current RN license.
  • Experience with case management in a hospice or similar setting; you need to know how to run the show for your patients.
  • The skill to assess both physical health and psychosocial needs to build an effective care plan.
  • You have to follow infection control standards strictly; it's non negotiable.
  • Knowing how to evaluate medical equipment for safety and anticipate supply needs is key.
  • You'll be collaborating with facility staff and family members constantly, so clear coordination is everything.

The details on pay & benefits

The salary range is $108,000 to $120,000 per year. There's also a sign on bonus up to $15,000 depending on the specific position.
