
About This Role
What You'll Do
You'll deliver physical therapy interventions to patients under the supervision of a Physical Therapist. Following the established PT plan of care, you'll provide treatments and educate patients and their caregivers. You'll also modify specific treatment procedures as the plan of care allows and within the PTA scope of practice.
Documentation is part of the job ; you'll record all care provided and keep those records according to VNS Health policy. You'll monitor patient progress through observation and follow up with the supervising therapist. A co visit with the PT is required on all initial PTA visits, and the PT will reassess each patient every 6 visits or 30 days, whichever comes first.
- Manage your assigned caseload and provide physical therapy services as directed by the PT's plan of care
- Participate in interdisciplinary team meetings, in service and training programs, rehab services meetings, and caseload reviews
- Travel to patients' homes and other facilities ; think elevated buildings, walk ups, care facilities, single or multiple family homes, places with pets ; using approved transportation options
- Help patients with therapeutic interventions and activities of daily living, which may involve positioning, moving, transferring, and lifting patients of various body sizes and physical conditions
- Instruct and verify capabilities of family members or paraprofessionals as needed
- Use and assist patients with durable medical equipment like wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise equipment
- Provide care in line with Agency policies and professional practice standards ; this means standing, stooping, sitting, crouching, twisting, reaching, bending, and stretching
- Transport a tablet, infection control materials, and supplies in a designated carrying case (backpack) weighing about 15–20 lbs to and from patient homes, care facilities, VNS Health offices, and your own home
- Take on special projects and other duties as assigned
What You Need
You need a New York State certification as a Physical Therapist Assistant. An Associate's degree as a PTA from a New York State approved program ; or an equivalent that New York State has approved ; is required.
Two years of experience in clinical patient care is required, per NY State Education Law. Bilingual skills might be required depending on operational needs. Basic PC skills are preferred. A valid driver's license may be required depending on the region.
Schedule and Pay
The pay range for this role is USD $33.88 to $42.35 per hour.
