Behavioral Health Technician

The Role

VNS Health is hiring a Behavioral Health Technician for the WeCARE Wellness Program in Bronx, NY. This program is funded by the Human Resources Administration. It's designed for clients with medical or behavioral health conditions who are temporarily unable to work.

After a clinical assessment, eligible clients get assigned to a care manager. You'd maintain a caseload of 70 clients and provide monthly follow ups. The wellness plan typically lasts 90 days, but it can be extended if needed. At the end of the plan, on site physicians review medical documentation to decide if the client can move into vocational rehabilitation, needs an extension, or should be considered for Supplemental Security Income.

You'll work under general supervision and coordinate services between various providers for clients with complex psychiatric, substance use, or co morbid medical conditions. It's about ensuring access to physical and behavioral health services plus community based resources that address social determinants of health.

What You'll Actually Do

• Use approved assessments to identify what clients and their families need; then develop an initial and ongoing clinical plan of care. Update that plan at specified intervals or whenever something changes in their condition or circumstances.
• Manage your assigned caseload effectively. Lead care coordination for complex psychiatric cases.
• Track progress and keep detailed, accurate progress notes and other documentation ; all updated in real time.
• Provide supportive counseling or supportive therapy along with ongoing mental health services.
• Collaborate with outside agencies and make referrals when necessary.
• Address client concerns to make sure they're satisfied with services. Use motivational interviewing techniques to encourage behavioral changes.
• Build an inventory of resources that match what each client needs based on their assessment.
• Link clients to service providers, coordinate with those providers, make referrals, and follow up ; including with managed care plans.
• Facilitate periodic case record reviews and case conferences involving every provider serving the client.
• Advocate for clients and educate them (and their families) on entitlement programs and community services they may qualify for.
• Arrange transportation and accompany clients to appointments when needed.
• Help clients or their families build a sustainable support network using their strengths ; tools designed to prevent future crises or reduce the damage if one does happen.
• Participate in initial trainings and ongoing ones so your clinical skills stay sharp.
• Maintain updated case records in the program's electronic medical record (EMR), following all policies, VNS Health standards, and regulatory requirements.
• Sit in on case conferences, staff meetings, utilization review sessions, and discharge planning meetings with your supervisor to determine if a client needs a different level of care or is ready for discharge.
• Be part of a 24/7 on call coverage schedule; you'll perform on call duties as required.
• Act as a liaison with other community agencies when needed.

The Program Team

The WeCARE Wellness Program isn't just you working alone. There's support from wellness health coaches who do outreach, run workshops, and help out the care management team. An operations team makes sure everything runs smoothly. Licensed social workers (Qualified Health Professionals) complete clinical assessments and manage higher need clients. And there's a leadership team made up of program coordinators, an operations supervisor, and directors overseeing it all.
