The Work You'll Do
You're the coordinator of care for patients dialyzing at home. That means collaborating with the whole health team to make sure everything is covered.
A huge part of this is being a teacher and an advocate for your patients. You'll help them get the information they need to give true informed consent for their treatment. Your goal is to support them in reaching the highest level of independence they want.
You'll also assess home environments. This involves driving to patients' homes to see if the space is suitable and safe for home dialysis, making sure it all meets CMS regulations.
On the professional side, you’ll be involved in your own development; things like continuing education, quality improvement projects, and applying research to your practice. We need someone who understands ethical practices and can navigate those challenges confidently.
What You Need to Bring
- A current, active, and unencumbered RN license for the state.
- At least one year of experience working as a Registered Nurse.
- Specifically, one year of nephrology nursing with dialysis experience. This needs to be in peritoneal dialysis, home hemodialysis, or a mix of both.
- A current Healthcare Provider CPR certification.
- You should be comfortable using Electronic Health Records; you need competent, in depth knowledge here.
- The ability to organize your work, prioritize tasks, and run with projects on your own.
- A valid driver's license and reliable transportation. Those home visits are a core part of the job.
