The Role
This isn't your typical nursing ad. If you're a Registered Nurse looking to move into Case Management, St. David's Medical Center has a program built specifically for that transition. The Program
They call it the New to Specialty Case Management Program, and it's designed for RNs who want to step into something new without being thrown into the deep end. Here's what it includes:
- A paid, immersive 6 week onboarding program with hands on learning and mentorship from experienced Case Management professionals
- 43 hours of Continuing Education Units (CEUs) that count toward your nursing license
- The organization pays for the American Case Management Association (ACM) certification exam and all required study materials ; no out of pocket cost for you
This program removes the financial barriers and gives you a strong foundation. It's for RNs ready to build specialized expertise in care coordination, patient advocacy, and outcomes management.
What You'll Actually Do
- Guide patients and families through program orientation, explaining rehab philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
- Coordinate education for patients, families, and caregivers so they can participate in treatment planning, goal discussions, and family conferences
- Develop and implement individualized treatment plans that reflect each patient's strengths, needs, and personal recovery goals
- Complete psychosocial assessments and discharge planning evaluations, documenting clearly and communicating needs across the care team
- Collaborate with the Rehab Program Director, Facility Case Management Director, and the full interdisciplinary team to deliver seamless, patient centered care
- Promote HCA Healthcare's values of compassion, respect, and excellence in every patient and family interaction
Qualifications
- Active Registered Nurse (RN) license
- Bachelors Degree, Masters Degree, or RN Diploma
- 3 years of nursing experience required
- No travel required for this position
