Alright, let's talk about this opening. We're looking for a Patient Care Manager for our home health team in Atlanta, GA. This isn't just an admin job; you're the person who makes sure our nurses and therapists have what they need to do great work, and that our patients are getting the right care at home. It's about leading the team that leads the care.
What You'd Be Doing
You'll be the go to clinical leader for your team. That means a few things.
- Providing clinical supervision to make sure all patient care meets professional standards, our own agency policies, and every law and regulation out there.
- Your day involves coordinating, planning, and monitoring patient care plans.
- Supervising the clinical personnel so services are delivered consistently and appropriately.
- Talking with patients, their families, referral sources, field staff, and office staff; you're the link that keeps communication clear.
- Working on quality outcomes. You'll help meet departmental goals and jump into quality improvement projects.
- How you show up matters. We need someone who models professionalism and our core values every day.
- There's a compliance piece here too. You'll complete required training, handle reporting accurately, and cooperate with any audits.
- Keeping an eye on safety for everyone, promoting good protocols.
- A big part of this is developing your team through coaching and supporting their continuing education.
The Stuff You Need
Here are the specifics we can't work around.
- You must be a registered nurse with a current license to practice in this state.
- A background in home care is important. Having about two years of management or supervisory experience is strongly preferred.
- You should know accepted professional standards and practices inside out. Familiarity with Medicare Conditions of Participation and other federal, state, and local regulations is key for this role.
The Schedule & Pay
This is a full time position. Your hours would be Monday through Friday from 8 a.m. to 5 p.m.The salary range is $110,000 to $135,000 per year. Where you land in that range depends on your experience.
