The Role
House Calls of New Mexico ; an Addus family company in Albuquerque, NM ; is looking for a full time Nurse Practitioner to join our team. This isn't just a job; it's a chance to really make a difference in our patients' lives. You'll be at the center of care, doing skilled nursing assessments, planning out patient care, and working to maximize symptom relief. Figuring out who's eligible for our services will be a big part of what you do.
You'll also be the person who identifies what patients and their families need, then provides the supportive care and appropriate services. All of this happens within the framework of an interdisciplinary team plan of care and our agency's policies.
What You'll Be Doing
- You take the lead on determining patient eligibility for care, using State and Federal guidelines as your guide.
- Gathering data on physical, psychological, social, and spiritual factors that affect a patient's and family's needs. When something comes up, you'll get the right members of the interdisciplinary team involved.
- You're the communication hub ; talking with the attending physician, other interdisciplinary staff, and outside agencies to coordinate admissions, recertifications, and the best care possible.
- Using resources effectively to meet patient needs, whatever that looks like.
- Making regular visits to reassess a patient's eligibility, their changing needs, and updating the plan of care as you go.
- Performing therapeutic, preventative, and rehabilitative nursing procedures ; but only as directed by the attending physician.
- Doing the initial assessment on every individual referred for care, deciding if they meet the criteria, and handling the admission.
- Keeping an accurate clinical record from day one ; care plans, clinical notes, and progress notes all get your attention.
- Staying in constant communication with the whole interdisciplinary team. You'll let the physician and nursing supervisor know about any changes in a patient's condition, needs, or any problems that pop up.
- Teaching the patient's family, caregivers, and the interdisciplinary team members so the care stays consistent and strong.
What You Need to Bring
- A graduation from an accredited registered nursing program.
- Palliative or Hospice experience is preferred but definitely not required.
- A license as a Nurse Practitioner in the state where you'll practice, plus 1 to 2 years of experience in that role.
- A minimum of two years of varied work experience as a professional nurse.
- A current CPR Certification ; you've got to have it.
- Real skill in nursing practices and solid clinical judgment.
- The ability to make home visits, which means providing nursing care in someone's home environment.
- Being able to cope with family emotional stress and tolerate different lifestyles.
- Good interpersonal, verbal, and written communication skills to get the job done.
- Reliable transportation, a current driver's license, and appropriate automobile insurance. Or you can take public transportation. Either way, you need to get there.
