About This Role

Alright, let's talk about this Service Coordinator role at Ambercare in Albuquerque, NM. It's not a remote gig; you're based out of the office on Osuna Road. Your main thing is making sure the care we promise actually happens for our clients. That means you're the point person for schedules, quality checks, and solving problems so older adults and people with disabilities can stay safe at home.

The Day to Day

You'll build and manage the schedules for our clients and the Home Care Aides who support them. When someone calls in sick or a client needs a change, you're finding the coverage so their care plan doesn't miss a beat.

  • A big part of your week is communicating updates to both care providers and clients.
  • You monitor how clients are doing and how the aides are performing their duties.
  • We need you to conduct home visits. You'll welcome new clients, review their welcome packets, and observe aides in the home to ensure care plans are followed correctly.
  • After those visits, you're writing up accurate evaluation reports and case notes.
  • Keeping on top of your schedule is key to getting all assigned home visits done on time.

You'll also be in our weekly branch meetings. And because you handle sensitive client information, maintaining strict confidentiality is non negotiable. You’ll follow all the rules; Medicare, Medicaid, HIPAA, company policies, everything we have.

What You Need to Bring

We need someone who's done this kind of work before. You should have at least three years of experience in senior services or a related social service field.

  • Your communication has to be solid, both talking with people and writing things down.
  • You're good with people and can build relationships easily.
  • The phone rings a lot here, so being able to prioritize tasks and handle a high call volume is important.
  • A client focused mindset drives everything you do.
  • Be comfortable with Microsoft Word, Excel, and Outlook.
  • This job requires travel for home visits within our service area. You must have reliable transportation, a valid driver’s license, and current auto insurance.

Schedule & Pay

The regular hours are Monday through Friday from 8:00 AM to 5:00 PM. But fair warning: per business needs, there will be some occasional evenings and weekends required. The pay is based on your experience. On top of that, there's a monthly agency bonus potential if we hit our goals together.
