What this job is
You're a nurse who's seen hospice care from the inside. Now, you're the one connecting patients and families to that care when they need it most. This isn't about sitting behind a desk; it's about being out in the community, in hospitals and facilities, making sure the transition to hospice is clear, compassionate, and seamless.
You'll handle everything from the first referral call to getting a patient admitted. That means assessing eligibility, explaining what hospice really means to scared families, and making sure all the clinical and paperwork boxes are checked. Your real skill is building trust; with discharge planners, case managers, doctors, and of course, with patients themselves.
What you'll do here
- Take referrals from hospitals, home health agencies, doctors' offices, skilled nursing facilities; you're often the first voice someone hears from us.
- Figure out if hospice is the right fit by reviewing medical records and talking with physicians and families.
- Spend time educating patients and their loved ones. You'll break down what hospice care involves and provide some much needed reassurance.
- Handle all the intake coordination and documentation so admissions happen smoothly.
- Act as the main link between our referral partners and our internal hospice care team.
- Get out of the office. You'll make daily visits to hospitals and facilities to stay visible and accessible to your contacts.
- Cultivate those professional relationships. Part of your role is finding new ways to raise awareness about our services.
The background you need
A current RN license for the state where you'll work is non negotiable. So is CPR certification. You also need a valid driver's license and auto insurance because this job gets you on the road.
We need someone with at least three years of hands on hospice experience. Five years or more puts you in a strong position. If some of that time was spent as a discharge planner or case manager, even better.
On paper: we require a Licensed Nurse with three to five years in hospice. A BSN degree paired with five plus years of experience is our ideal combination.

The pay & benefits
The salary range for this position is $68,000 to $85,000 per year. Where you land in that range depends on things like your specific experience, education, certifications, skillset; and yes geography matters too (that's why we use Akron, OH). Base pay is just one part of what Gentiva offers though.
